ADDITIONAL CHECKS:

47

Did you spell check and proofread the report before sending it to 21 OG?

48

Did you verify print quality (fading, streaks, etc.) before sending the report to 21 OG?

-  NOTE:  White-out is not acceptable.

49

For the draft report, did you verify the report reached 21 OG NLT 30 days prior to closeout?  If not, submit a late letter in memorandum format to 21 OG/CC.

50

Did you include a copy of the individual's last 3 OPRs?  (Exception:  Since 2LT and 1LTs may not have 3 prior OPRs, include the training reports in the absence of OPRs.)

51

For the final report, did you verify the report reached 21 OG NLT 5 days after it was returned for corrections?

52

Did you review the document for the following COMMON ERRORS?

1.       Rater didn’t hand write the actual date the report was signed after the closeout date.

2.       Unit must be spelled out in signature element for indorser/raters (i.e. 21st Space Wing)

3.       Unit must annotate (if shell and OPR do not match) that changes were made in PC-III. When changes are made, provide the PC-III source document showing the change.

4.       Keep previous changes made by the wing command section in folder when it is returned.  (N/A for GSUs except 1 CACS.)

5.       Bullet statements not aligned. Check spaces between words. Only one space is needed. Use bullets in Section III, block 2, and Sections IV, V, VI, and VII.

6.   When a report is returned for corrections, ensure the raters use the actual date the

report is signed and not use the date on the original report.  Rater must hand write

      dates.

7.   Although the AFI states that white-out is acceptable, 21 SW will not accept any reports

      with white-out.

8.    Primarily write OPR in past tense, except for the promotion recommendation bullets.

9.    Words that require hyphens are typed without the hyphen or words that do not require a

hyphen are typed with the hyphen.  Do not capitalize the first word after a hyphen.

10.  Acronyms are typed without typing the meaning or acronyms are typed but not referred

       to again in the text.

11.  The words “wing” and “group”, typed within a sentence, should be capitalized only when

they refer to an actual organization.  (Ex:  21 Space Wing, 21 Operations Group,

       operational wing, wing inspection…)

12.  Do not capitalize the first character of a word after the double dash (i.e. --promote)

13.  Organizational symbols are typed incorrectly (i.e. 21 SW, not 21st SW)

14.  Use “Additional Duty” when only one duty is listed, and “Additional Duties” when more

than one duty is listed.  Can also use Significant Additional Duty(ies) if appropriate.

15.  Not using bullets in all sections (except Unit Mission Description).

16.  Illegal entries in report (e.g., enrollment or completion of an advanced academic

degree, enrollment or completion of PME, overt or veiled promotion statements.)

17.  Forgetting to include only pg 5 of the OPR Checklist signed by the sq CC.
